2 ）提供所投设备原厂 DATASHEET 。

3 ） 如果投标人所投的货物不是投标人自己制造的，投标人应得到腹腔镜系统制造商同意其在本次投标中提供该货物的正式授权书（参见第一分册格式 IV-9-4 ）和制造商资格声明（参见第一分册格式 IV-9-2 ） 。

1) Provide the complete medical device registration certificate of the bidding product.

2) Provide the original factory DATASHEET of the invested equipment.

3) If the goods submitted by the Bidder are not manufactured by the Bidder the Bidder shall obtain the approval of the manufacturer in this bid (see Sub-volume format IV-9-4) and the manufacturer's qualification statement (see Sub-volume format IV-9-2).
